The first step toward enrollment is filling out a simple application. Don't worry, we're here to help.
Registration
Parents desiring to place their child(ren) into CCS should notify the school office and request the necessary forms.
The school office will then supply the necessary application forms along with a packet of statements and policies. The parents will be required to complete the forms and submit them to the school along with the appropriate registration and book fees. Applications are not considered complete without the registration fees.
Upon receiving the application forms and registration fees, the administrator will call and set up a family interview. It is ideal if both parents and the prospective student can be in attendance.
The school board will then review the application, and a letter of acceptance (or non-acceptance) will be sent to the parents as soon as possible.

Children's Scholarship Fund of Omaha: Visit their website for more information at www.csfomaha.org to see if you qualify to apply for Tuition Assistance. Qualified applicants can apply from February 2024 until May 2024. Please read all information before you begin the application process.
Enterance Requirements
Upon acceptance, the following requirements will be due immediately:
- Registration Fee turned in with Application Form
- Birth Certificate (all students PreK through 8th Grade)*
- Previous school records (transferring students)
- Health Forms including the following:
Preschool:
Immunization Records** & Signed DHHS Form (Given by the Preschool Teacher)
All Students:
Immunization Records**
Entering Kindergarten:
Current Immunization Record**, Eye Examination, Physical Examination
Transferring Students:
Immunization Records**, Copy of Kindergarten Physical and Eye Exams
- This completes the registration process. Students cannot be admitted to the classroom until all the requirements have been met. * If you do not have a certified Birth Certificate
** For parents who choose not to immunize their children, an affidavit must be filled out and turned into the school office
